What is a Community Health Worker?

A Community Health Worker (CHW) is a trusted member of the community who has a strong understanding of community resources. If you are in need, a CHW can assist you with social services and navigating the healthcare system. At Trinity Health, we understand that concerns like your rent or mortgage, the price of food, reliable transportation, and other worries, along with rising prices, can have a significant impact on your health and quality of life.

In other words, CHWs work closely with you to address non-medical needs and other concerns that may impact your ability to manage your health.

This program is free and completely confidential for patients, community members, and colleagues. 

What is?
How Can They Help?

When you first talk with a CHW, they will work with you to understand what you need and partner with you to find free or reduced-cost resources.

​

A community health worker can:
• Link you to community services and resources for help with: food, language, transportation,
       insurance, and housing
• Help you find medical care for your needs (primary, dental, specialty, mental health, etc.)
• Help you manage your health conditions and prescriptions
• And more!
